Effects of expressive writing intervention for women's PTSD, depression, anxiety and stress related to pregnancy: a meta-analysis of randomized controlled trials

Across 8 randomised trials with 929 women, expressive writing was an effective therapy for reducing pregnancy-related post-traumatic stress - but it did not produce the expected effects on anxiety or stress symptoms, and its effect on depression was inconclusive.

Qian and colleagues searched five databases and pooled eight randomised controlled trials covering 929 women, testing expressive writing against pregnancy-related post-traumatic stress, depression, anxiety and stress.

The pooled analysis found expressive writing was an efficient therapy for decreasing PTSD. For anxiety and stress, the intervention was not associated with the expected effects. For depression, the results were inconclusive.

That split matters more than a single headline number. The expressive writing paradigm was built to help people process a specific difficult event, and this is where it performs - not as a general-purpose anxiety tool. A journal is not an anxiolytic, and this is the cleanest evidence of that distinction available.

The authors’ methodological recommendation is worth noting for reading any writing study: trials should include a neutral writing group and a no-writing group, because otherwise you cannot tell whether the benefit came from the emotional content or simply from writing at all.

What this study does not show

Only the PTSD result held. Anxiety and stress showed no expected effect, and depression remained inconclusive on this evidence. Eight trials is a thin base, and the authors call for more randomised trials plus better-designed comparisons using both a neutral writing group and a no-writing group.
